RESUMEN Objetivo: determinar la frecuencia, incidencia y describir las características clínico- epidemiológicas de los pacientes con diarrea nosocomial por Clostridioides difficile en el Hospital Nacional Edgardo Rebagliati Martins en el período abril a julio de 2019. Materiales y métodos: estudio observacional y transversal realizado en los servicios de medicina interna, cuidados intensivos, hematología, gastroenterología y emergencia. Se realizó una vigilancia activa de los pacientes mayores de 18 años con tres o más deposiciones no formadas en 24 horas luego de 48 horas de admisión hospitalaria. Se empleó un algoritmo diagnóstico, que incluyó la glutamato deshidrogenasa y toxina fecal A y B de C. difficile. En caso de resultados discordantes, se realizó la prueba de amplificación de ácidos nucleicos de los genes productores de toxinas. Resultados: de 107 pacientes con diarrea nosocomial, 12 fueron positivos para C. difficile con una frecuencia de 11,2% (IC 95%: 6,44-18,82). Un paciente fue NAP1-BI-027 positivo. La mayor densidad de incidencia fue en hematología (5,54 x 10 000 pacientes-día). Siete (7/12, 58,3%) fueron varones y la edad fue 64,58 ± 21,34 años. El principal antibiótico indicado previo a la diarrea fue meropenem (9/12, 75%). Solo dos pacientes recibieron clindamicina. La mayoría (8/12, 66,7%) tuvo un episodio inicial no grave. La mortalidad general fue 28% (IC 95%: 20,27-37,38) y cuatro fallecidos tuvieron infección por C. difficile. Conclusiones: la frecuencia de diarrea nosocomial por C. difficile en nuestra institución fue menor al reportado previamente en otro hospital público de Lima. El uso racional de antibióticos y las medidas preventivas explicarían estos hallazgos.


ABSTRACT Objective: to determine frequency, incidence, and to describe clinical and epidemiological characteristics of patients with nosocomial diarrheal disease caused by Clostridioides difficile in Edgardo Rebagliati-Martins National Hospital in Peru, between April to July 2019. Material and methods: this is an observational and cross-sectional study that was performed in Internal Medicine, Intensive Care, Hematology, Gastroenterology, and Emergency services. Active surveillance of patients more than 18-years old who had three or more non-formed stools in a 24-hour period at 48-hours after being admitted to the hospital. A diagnostic algorithm was used, including glutamate dehydrogenase and A and B C. difficile fecal toxins. In case of discordant results, nucleic acid amplification for toxin producing genes tests were performed. Results: out of 107 patients with nosocomial diarrheal disease, 12 were positive for C. difficile, with 11.2% frequency (95% CI: 6.44 - 18.82). One patient was NAP1-BI-027 positive. The maximum incidence was found in the hematology service (5.54 x 10000 patients- day). Seven (7/12, 58.3%) patients were male and average age was 64.58 ± 21.34 years. The most frequently prescribed antibacterial agent prior to the diarrheal disease was meropenem (9/12, 75%). Only two patients had received clindamycin. Most patients (8/12, 66.7%) had an initial non severe episode. Overall mortality was 28% (95% CI: 20.27-37.38) and four deceased patients had C. difficile infection. Conclusions: The frecuency of nosocomial diarrheal disease caused by C. difficile in our institution was lower than that reported in another public hospital in Lima. Rational use of antibacterial agents and preventative measures may explain these findings.

Introducción. Las enfermedades digestivas (ED) no neoplásicas son causas importantes de consulta médica y hospitalización. Objetivo. Determinar la mortalidad por ED no neoplásicas en la población adulta del Perú entre 2010 y 2015. Métodos. Análisis secundario del registro de defunciones por ED no neoplásicas en la población mayor de 18 años entre los años 2010 y 2015 de la Oficina General de Estadística e Informática del Ministerio de Salud del Perú. La población para el mismo periodo se obtuvo del Instituto Nacional de Estadística e Informática. Se calcularon las tasas de mortalidad estandarizada (TME x 100 000 personas-año). Resultados. Durante el período 2010-2015 se registraron 39 889 muertes por ED no neoplásicas. Las TME más altas fueron para el grupo de las enfermedades del hígado (12,2 x 100 000 personas-años), siendo la fibrosis y cirrosis del hígado la de mayor mortalidad (9,1). La pancreatitis aguda (1,0) tuvo mayor TME que la úlcera gástrica (0,7). Las TME, según las regiones del Perú, fueron altas por fibrosis y cirrosis del hígado en Piura (16,3), Lambayeque (14,6) y La Libertad (14,4); por colelitiasis (1,1) y colecistitis aguda (1,7), en Puno; por pancreatitis aguda (2,2), en Tacna; y por úlcera gástrica fue mayor en Huancavelica (3,6) y Huánuco (2,3). Conclusiones. La fibrosis y cirrosis del hígado fue la ED no neoplásica con mayor tasa de mortalidad en la población adulta del Perú, principalmente en la costa norte del país.


Introduction. Non-neoplastic digestive diseases (DD) are important causes of medical consultation and hospitalization. Objective. To determine the mortality by non-neoplastic DD in the adult population of Peru between 2010 and 2015. Methods. Secondary analysis of the registry of non-neoplastic DD deaths in the population over 18 years old between 2010 and 2015 of the General Office of Statistics and Informatics of the Ministry of Health of Peru. The population for the same period was obtained from the National Institute of Statistics and Informatics. The standardized mortality rates (SMR x 100 000 person-years) were calculated. Results. During the period 2010-2015 there were 39 889 deaths due to non-neoplastic DD. The highest SMR were for the group of liver diseases (12,2 x 100 000 person-years), with fibrosis and cirrhosis of the liver having the highest mortality (9,1). Acute pancreatitis (1,0) had higher SMR than gastric ulcer (0,7). The SMR, according to the regions of Peru, were high due to fibrosis and cirrhosis of the liver in Piura (16,3), Lambayeque (14,6) and La Libertad (14,4); by cholelithiasis (1,1) and acute cholecystitis (1,7), in Puno; for acute pancreatitis (2,2), in Tacna; and gastric ulcer was higher in Huancavelica (3,6) and Huánuco (2,3). Conclusions. Liver fibrosis and cirrhosis was non-neoplastic DD with a higher mortality rate in the adult population of Peru, mainly in the northern coast of the country.

Objetivo: Determinar la percepción del gastroenterólogo sobre las necesidades de educación médica continua (EMC) en el Perú. Material y métodos: Estudio transversal y descriptivo. La muestra fue no probabilística. Se aplicó una encuesta a los gastroenterólogos miembros de la Sociedad de Gastroenterología del Perú. El cuestionario se elaboró en base al "Canadian Association of Gastroenterology Educational Needs Assessment Report" con una escala tipo Likert de 5 puntos (1= no necesario y 5= indispensable). Se determinó el promedio de los puntajes obtenidos en cada uno de los 33 ítems de las áreas clínica, endoscópica y métodos de aprendizaje. Resultados: Hubo 75 participantes y la edad promedio fue 43,40 años (DE ± 10,22 años). El lugar de trabajo principalmente fue Lima (68%) y la mayoría (50,67%) tuvo un tiempo de servicio menor de 5 años. La percepción de necesidades educativas en el área clínica fue mayor para el cáncer gástrico (4,37 ± 0,87) y el cáncer de colon (4,37 ± 0,83); en el área endoscópica fueron la polipectomía (4,15 ± 0,95) y las técnicas de emergencia (4,13 ± 0,99). Los principales métodos de aprendizaje para los gastroenterólogos fueron la asistencia a congresos (4,29 ± 0,83) y los talleres endoscópicos (4,19 ± 1,06). Conclusiones: La percepción del gastroenterólogo encuestado sobre las necesidades de EMC fue principalmente sobre temas de cáncer gástrico y de colon. La mayoría de ellos consideró la asistencia a congresos como el principal método de aprendizaje.

Most of them considered congress attendance as the main learning method.

The field of action of gastroenterology has been expanded due to technological development and the advent of new sub-specialties, such as gastroenterology oncology. Currently, there is no standardization of medical training programs in gastroenterology in our country. The health system and education are changing, so medical practice and competency assessment for medical certification and recertification should reflect these changes. On the other hand, the quality of a specialized unit, service or medical department is directly related to the quality of human resources. Lifelong learning is reflected in continuing medical education (CME). The goal of CME should be to achieve changes in staff conduct, through continuous improvement in daily practice. This requires knowing the social, institutional and individual needs and developing new, more flexible and individualized CME programs. Recertification at fixed intervals should be abandoned in favor of a model that promotes continuous professional development based on health needs and with curricular materials that support competency assessments.

OBJECTIVE: To determine the prevalence of Helicobacter pylori in symptomatic outpatients in Red Rebagliati (EsSalud) from 2010 to 2013. MATERIAL AND METHODS: An observational, transversal and analytical study. Records of outpatients with diagnostic 13C-urea breath test of Helicobacter pylori in the period 2010-2013 were reviewed. RESULTS: Of the 1711 patients, the overall prevalence of Helicobacter pylori was 45.5% (95% CI: 43.17-47.89) being higher in women than in men (47.1% vs. 42.1%, p=0.056). There were significant differences in age between infected and uninfected (44 vs. 39, p < 0.05), with association between age and the presence of Helicobacter pylori (p<0.001). The estimated prevalence in the pediatric population was 36.3% and was found positive in 201 (51.1%) women of childbearing age. Most (43.9%) came from the financial sector of the city. CONCLUSIONS: The prevalence of Helicobacter pylori in the study population was similar in both sexes and is related to age. The average socioeconomic status of Lima maintains the prevalence of Helicobacter pylori reported in the last years.

Determinar la prevalencia de Helicobacter pylori en pacientes sintomáticos de consulta externa en la Red Rebagliati (EsSalud) en el período 2010-2013. Materiales y métodos: Estudio observacional, transversal y analítico. Se revisaron los registros de pacientes ambulatorios con la prueba en aliento con urea-13C diagnóstica de Helicobacter pylori en el período 2010-2013. Resultados: De los 1711 pacientes, la prevalencia global de Helicobacter pylori fue 45,5% (IC 95%: 43,17- 47,89), siendo mayor en mujeres que en varones (47,1% vs. 42,1%, p=0,056). Hubo diferencias significativas en la edad entre infectados y no infectados (44 vs. 39, p<0,05), con asociación entre la edad y presencia de Helicobacter pylori (p<0,001). La prevalencia estimada en la población pediátrica fue 36,3% y se encontró resultado positivo en 201 (51,1%) mujeres en edad fértil. La mayoría (43,9%) procedían del sector financiero de la ciudad. Conclusiones: La prevalencia de Helicobacter pylori en la población de estudio fue similar en ambos sexos y tuvo relación con la edad. El nivel socioeconómico medio de Lima mantiene la prevalencia de Helicobacter pylori reportada en los últimos años...

RESUMO

The inhabitants of the peruvian-bolivian plateau consume a natural substance known as "Chaco", widespread since pre-Columbian era and appreciated for its digestive properties. The Chaco is an edible medicinal clay that is used as slurry with water to restrain dyspeptic discomfort or acid-peptic manifestations. In this contribution we present physicochemical aspects of the composition of the Chaco, experimental animal studies that evaluate its antiulcer effect and in vitro test that studies the antacid property. The proposed mechanism of therapeutic action is due to a cytoprotective effect on the gastric mucosa by independent mechanisms of acid secretion inhibition, as it has no antacid property in vitro. Also it has an adsorptivity to different organic molecules due to their large surface area and tetrahedral charge that makes it to interact with polar substances such as water and toxins. The other purpose of this special contribution is to recognize the coexistence of "Traditional Medicine" and "Western Medicine", a situation which leads to the need for preclinical research of various natural resources.

Los pobladores del altiplano peruano-boliviano consumen una sustancia natural conocida como "chaco", muy difundida desde la época precolombina y apreciada por sus propiedades digestivas. El Chaco es una arcilla medicinal comestible que es usada en forma de suspensión con agua para cohibir molestias dispépticas o manifestaciones ácido-pépticas. En esta contribución damos a conocer aspectos físico-químicos de la composición del Chaco, estudios experimentales en animales que evalúan su efecto antiulceroso y una prueba in vitro que estudia su propiedad antiácida. El mecanismo de acción terapéutico propuesto se debe a una acción citoprotectora sobre la mucosa gástrica por mecanismos independientes de la inhibición de la secreción ácida, ya que no posee propiedad antiácida in vitro. Además tiene una capacidad de adsorción a distintas moléculas orgánicas debido a su gran superficie externa y carga tetraédrica que hace que interaccione con sustancias polares como el agua y toxinas. El otro propósito de esta contribución especial, es reconocer la coexistencia de la "Medicina Tradicional" y la "Medicina Occidental", situación que conlleva a la necesidad de la investigación preclínica de diversos recursos naturales.

We report a 61-year-old woman who was hospitalized because of abdominal pain in the right upper quadrant related to a liver tumor (ultrasound and tomographic findings). A collection of blood was obtained by a biopsy and there were no tumor cells. With the suspicion of acute fascioliasis (liver stage), due to severe eosinophilia and recent travel to endemic area of Fasciola hepatica, arc II and ELISA Fas 2 we carried out and were positive. Parasitological stool examinations were negative. During hospitalization a hepatic subcapsular hematoma presented as a complication and the patient developed fever because of cholangiolitic microabscesses in the left hepatic lobe. Percutaneous drainage was performed and positive cultures of secretions were obtained She received antibiotic coverage with vancomycin and imipenem. Treatment for Fasciola hepatica was initiated with nitaxozanida but it was discontinued due to oral intolerance. Later, she received a single dose of 250 mg triclabendazole with clinical and laboratory improvement. We presented this case because it is an unusual pseudotumoral presentation in acute hepatic fascioliasis. This parasitic disease is an emerging zoonosis in Perú.

We presented this case because it is an unusual pseudotumoral presentation in acute hepatic fascioliasis. This parasitic disease is an emerging zoonosis in Perú.

Se reporta el caso de un paciente varón de 69 años con un gran adenoma de las glándulas de Brunner que se presentó con anemia severa sintomática y pérdida de peso significativa. El estudio endoscópico alto detectó una gran tumoración duodenal que se extendía del bulbo hasta la segunda porción y la biopsia fue informada como duodenitis crónica inespecífica con cambios metaplásicos gástricos. El paciente fue sometido a una duodenopancreatectomía y la histología de la pieza quirúrgica fue un tumor de glándulas de Brunner. El adenoma de glándulas de Brunner se considera un tumor benigno del duodeno; sin embargo, la resección endoscópica está limitada a lesiones menores.


We report the case of a 69-year-old male patient with a large Brunner's gland adenoma, who presented with symptomatic anemia and significant weight loss. The upper gastrointestinal endoscopy showed a large duodenal tumor extending form the bulb to the second portion, and the biopsy was reported as chronic non-specific duodenitis with gastric metaplastic changes. The patient underwent a duodenopancreatectomy, and the histological study of the surgical specimen was reported as a Brunner's gland tumor. Brunne's gland adenoma is considered a benign tumor of the duodenum; however, endoscopic resection is restricted to minor injuries.
